If you would like to become a member of our congregation, please contact the pastor and/or the church office. Then, please fill out this form and return it to the Parish Administrator by email or regular mail.

BAPTISM FORM

If you would like to be baptized, or would like to have your child baptized, please contact the pastor or the church office. Then, please fill out this form and return it to the Parish Administrator by email or regular mail.

MEMORIAL PLANNING GUIDE

If you have lost a loved one and would like the memorial service to be held at Queen Anne Lutheran, please call Pastor Dan. He will set up a time to meet with you and your family. He will give you a copy of our Memorial Planning Guide, which is also available here:

FORMS FOR GIVING AND EVENT PLANNING

SIMPLY GIVING FORM

If you are a member and would like to set up regular, automatic payments to the church, please download this form. Put it in an envelope and mail it to the church office, ATTN: Financial Secretary, or bring it in to church.

BUILDING USE FORM

If you are requesting to use the building for an event please fill out the entire form. If you need to create a diagram of table set-up please include a sketch on the second page. Please fill out this form and return it to Parish Administrator by email or regular mail.

REQUEST FOR REIMBURSEMENT FORM

If you spent your own money for supplies or food related to a church program or event and wish the church to reimburse you, please fill out form below and attach your receipt to it. You can put both into the treasurer’s mailbox in the church office. After approval, your reimbursement check will arrive in the mail.

FORMS FOR VOLUNTEERS AND COMMITTEES

BACKGROUND CHECK AUTHORIZATION

If you are going to volunteer at our church and work with children or vulnerable adults, we will complete a background check for criminal history as part of our Safe Haven Policy. Please download the form below and bring it to the church office.  The form is shredded once the background check is done.

SOCIAL MEDIA POLICY
