Safe Haven at Queen Anne Lutheran

 

Every volunteer at Queen Anne Lutheran Church who works with or interacts with children needs to be aware of and abide by our Safe Haven Policy. Our online training currently consists of the following steps:

  1. Access the Safe Haven Policy by clicking here. Read it carefully, particularly pages 1-12. If you prefer to have a printed copy, they are available in the church office. Questions can be directed to Terry Anderson, our Children’s Ministry Coordinator.
  2. View this video link about the role of being a Washington State Mandatory Reporter. (6 minutes)
  3. Sign the acknowledgement that you have read the policy and will follow it, that you have viewed the video, and that you will abide by confidentiality rules that are in the policy. (Click here for the form; print the form out, sign it, and deliver it to the church office. Once it is signed, if you prefer to scan and email it to the office, you may do so. Note: Paper forms are also available from the church office.)
  4. Fill out a background check authorization form.(Click here for the form; print the form out, fill it out and sign it, and deliver it to the church office. Once it is signed, if you prefer to scan and email it to the office, you may do so. Note: Paper forms are also available from the church office.)
  5. The office will contact you once your background check is completed; the personal information part of the authorization form will be shredded, so your SSN will not be in the office files.
